In my current role I manage medical nutrition therapy for an inpatient and outpatient caseload, and a diabetes education program I built raised the share of my patients hitting their A1c target from 54 to 76 percent over a year. I would like to bring that same outcome focus to the Registered Dietitian role at [Company].
I am a Registered Dietitian Nutritionist (RDN) and licensed in this state, with five years across clinical, renal, and outpatient counseling settings. I complete nutrition assessments, write care plans using the nutrition care process, manage enteral and parenteral nutrition for complex patients, and chart in the EHR. I counsel patients in plain language and meet them where they are, because a perfect plan no one follows changes nothing. When a renal patient kept landing back in the hospital, I reworked their plan around foods they would actually eat and their readmissions stopped.
What I bring beyond clinical knowledge is the ability to make nutrition stick. I run group classes that fill, I coordinate with physicians and nursing as part of the care team, and I document thoroughly for both care and reimbursement. I keep current on evidence-based guidelines and translate them into advice a patient can use this week.
Getting three quarters of that panel to their A1c target took restructuring the education sessions around shift workers, and I would like to hear whether your outpatient population has the same problem. My RDN credential and state licensure are both active.
